Purpose
To assess the value of cardiac MRI in patients with acute chest pain, elevated cardiac enzymes, and a negative coronary angiogram.

In a 39 months period, 127 patients from the chest-pain unit (CPU) could be included in this study. Purpose
To assess the value of cardiac MRI in patients with acute chest pain, elevated cardiac enzymes, and a negative coronary angiogram.

Materials and Methods
In a 39 months period, 127 patients from the chest-pain unit (CPU) could be included in this study. The MR imaging protocol was kept as simple as possible and consisted of Cine, edema sensitive and late Gadolinium enhancement imaging. Clinical follow-up and the synopse of all clinical, laboratory, and imaging data were the basis for establishing a consensus based diagnosis. This diagnosis served as the standard of reference.

Results
Based on cMRI, there were 19 patients with acute myocardial infarctions, 25 patients with dilated cardiomyopathy (DCM), 47 with myocarditis, 14 with Tako-Tsubo cardiomyopathy and 28 with miscellaneous diagnoses. Compared with the final standard of reference diagnosis for the four main diagnoses, cMRI enabled establishing the final diagnosis in 89/99 patients (90%). Only in 10 patients, no final diagnosis could be established.

Conclusion
Cardiac MRI plays an important role in patients with acute chest pain, elevated cardiac enzymes and a negative coronary angiogram as it helps in establishing the final diagnosis in the vast majority of cases.
