Diagnostic and psychometric feedback within a pilot project on quality assurance in outpatient psychotherapy

Data was avai...This is the first study in Germany to evaluate a new quality assurance and feedback tool with regard to its practical feasibility in routine care and within the German health insurance system. In this paper, the effectiveness of outpatient psychotherapy under routine care, the effects of the implementation of a checklist to support the diagnosis, the effect of the feedback system based on psychometrics and the evaluation of the project by its participants (patients) are covered. Data was available for 1,708 patients treated by 245 therapists (intervention group: 1,031; control group: 677). Overall, the study demonstrated the effectivity of outpatient psychotherapy and its application to clinically impaired patients. The implementation of the more structured diagnostic tool (IDCL) had an impact on the distribution of diagnosis between groups as well as on the overall frequency in detecting comorbidity. With respect to feedback, it was possible to group patients (in accordance with previous results from the literature) based on their early feedback results into different predictive outcome groups. Most therapists used the feedback to adapt their treatment and discussed it with their patients. Additionally, the patients in the intervention group (IG) were more satisfied with the therapy (d=.27). The duration of the IG is well in the range of the duration in other control groups delivering the same service in different regions of Germany. The satisfaction with the project in the IG was very high for patients. The patients thought of the feedback system as an important feature of the therapy, acceptable in time needed, helpful, and in accordance with their own perception.
